Library Cards
A. Residents of Nephi, Mona, Levan, Rocky Ridge, Yuba Dam, Juab County (outside of Nephi City Limits), and teachers in the Juab schools may apply for a library card.
B. An annual fee of $20.00 will be charged per family for those who live outside the Nephi City limits. They will receive one card for their family to use.
The following is required at the time of application:
- Identification and proof of residence. A current driver’s license with local address, utility bill, rent receipt, etc. will be acceptable.
- Reference: The name and phone number of someone in the area who knows the applicant such as friends, family, neighbors, employer, or bishop
- The signature on the application card shows that the applicant has read the rules, understands them, and agrees to obey the rules of the library.
4. A parent or guardian’s signature is required for all children under the age of eighteen.
C. Nephi residents applying for a library card may choose an individual card of a family card.
D. Patrons are asked to give notice of change of address or phone number and loss or theft of card. Until notice is given, they shall be responsible for all materials checked out with their cards.
E. New card holders will be allowed to check out only one book at a time per family member for up to three months.
F. Librarians reserve the right to refuse “out of library” service to patrons who continually abuse the library rules.
