Circulation Policy
1. Basic Borrower Rules
A. Borrowers must have a valid library card
in order to check out library materials.
B. A fine shall be charged on each item loaned which
is not returned according to Library rules.
C. All injuries and damages to materials beyond
normal and reasonable wear, as well as lost items, must be monetarily covered or
the item replaced to the satisfaction of the librarians.
D. Borrowers shall be held responsible for all
materials checked out on their cards and for all fines accrued on their cards.
E. Parents or guardians shall be held responsible for
all materials checked out by members of their immediate family or under their
care.
2. Library Cards
A. Residents of Nephi, Mona, Levan, Rocky
Ridge, Yuba Dam, Juab County residents (outside Nephi City limits), and teachers
in the Juab schools my apply for a library card.
B. Residents of communities other than Nephi City
will be asked to pay a fee of $20.00 each year per family to use the library
services.
C. The following is required at the time of
application:
1. Identification and
proof of residence. (A current driver’s license with local address, utility
bill, rent receipt, etc. will be acceptable.)
2. Reference: the
name and phone number of someone in the area who knows the applicant (such as
friends, family, neighbor, employer, or bishop).
3. The signature on
the application card shows that the applicant has read the rules, understands
them and agrees to obey all of the rules of the library.
4. A parent or
guardian’s signature is required for all children under the age of eighteen.
D. Nephi residents applying for a library card may
choose an individual card or a family card.
E. Patrons are asked to give notice of any change
of address or phone number, as well as any loss or theft of their card. Until
notice is given, they shall be responsible for all materials checked out on
their cards.
F. New card holders will be allowed to check out
only one item at a time per family member, for up to three months.
G. Librarians reserve the right to refuse
circulation services to patrons who continually abuse the library rules.
3. Loan Period
A. Printed materials, audio books, computer software,
and CDs may be checked out for a period of two weeks, with one renewal of two
weeks.
B. The newest issues of magazines will not be allowed
to leave the library. Older issues may be checked out for a period of two weeks.
C. The most current sets of encyclopedias will not be
allowed to leave the library. Older sets may be checked out for a period of two
weeks.
D. Videos and DVDs may be checked out for one week,
with one renewal of one week.
E. Materials may be loaned to teachers for classroom
use and to other acceptable organizations for extended periods of time.
4. Fines and Fees
A. There will be a $1.00 charge for all new or
replacement library cards.
B. Fines of $0.10 per day per item will be charged
for overdue materials (excluding videos and DVDs).
C. Fines of $0.50 per day per item will be charged
for overdue videos and DVDs.
D. Fines shall not exceed $2.00 for each overdue
item.
E. Fines will not be assessed on days that the
library is closed.
F. Patrons will be charged the replacement cost of
lost or damaged materials.
G. If the library has to send several overdue
notices, a $2.00 fee will be added for the cost of postage, paper and ink.
H. If any immediate family member has an overdue book
or owes fines, service may be refused to the entire family until all charges are
paid in full and all items are returned to the library.
I. Delinquent patrons shall not be allowed to
check out additional materials until all charges are paid and all items are
returned to the library.
5. Overdue Materials
A. When materials are approximately ten days overdue,
notices will be sent at weekly intervals to remind patrons.
B. If there is no response, a bill will be sent for
the cost of the materials checked out and fines accrued. At this time, a $2.00
fee will be added for postage, paper and ink.
C. If there is still no response, the matter will be
turned over to our city attorney and a letter will be sent stating that if the
materials are not returned within ten days, the patron will be turned in to
small claims court.
D. If no response to the letter, the patron will be
served papers and will need to appear in court. They will have to pay all fines
to the library and all court costs to the Nephi City Court.
6. Reservations
A. Materials that patrons want that are checked out
of the library, can be reserved in the order they are requested.
B. When their name comes up on the reserve list, the
patron will be notified by phone. The reserved item will be held for 2-3 days.
If not picked up, it will go to the next patron on the list.
C. Patrons on reserve lists who have overdue books or
owe fines will need to return the books and pay their fees before they can check
out the item on reserve.
D. Patrons who want books that our library does not
have, can request such books through Inter-Library Loan and the patron will be
asked to pay return postage.
7. Non-Circulating Material
A. Material in the Special Collections Room will not
be allowed to leave the library.
B. The most recent sets of Encyclopedias and the
current issue of magazines will not be allowed out of the library.
